Mrs. Anita Sharma, a 75-year-old patient with a history of hypertension and osteoarthritis, has been admitted to the medical-surgical ward for post-operative care following a hip replacement surgery. She is alert but complains of occasional dizziness and weakness while attempting to stand or walk. Her mobility is currently limited, and she requires assistance for most activities of daily living. During the previous night shift, she attempted to walk to the bathroom without assistance despite being advised to call for help. As a result, she slipped and fell near her bedside, causing minor bruises on her arm. The incident was reported to the nursing supervisor, and an internal investigation revealed that no fall risk assessment was performed upon her admission. Additionally, the call bell was not within the patient’s easy reach. The fall has caused Mrs. Sharma significant emotional distress, making her anxious about her recovery. Her family is upset about the incident and has expressed concerns regarding the hospital’s safety protocols. The nursing staff is tasked with ensuring her safety, providing emotional support, and preventing similar events in the future.
